How to Qualify for Our Treatment Plan Options

Traditional IVF
Anyone can qualify for a traditional one-cycle IVF plan. However, your pre-cycle testing results will provide you with the information you will need to make an informed decision. The Center will help you understand any risks you are taking and what your individual probability for success might be.

Frozen Embryo Cycle
The Frozen Embryo Cycle is a fee-for-service cycle except when it is included in the specific plan you have chosen. If the plan calls for inclusion of a specific number of frozen embryo transfers, any transfer beyond that number will be performed on a fee-for-service basis. Only embryos derived from the fresh cycles in the patient’s plan may be used for frozen embryo transfer. No frozen embryos from cycles outside the plan can be transferred.

Assistance Program
The Assistance Program is a special program our Center offers to one couple per month who have a combined income of less than $50,000 per year validated by their IRS forms for the preceding two years. They must not have insurance coverage for infertility treatment and they must be U.S. citizens. Couples can participate in only one drawing.

Joint Venture Plan
In the Joint Venture Plan, both the couple and the Center are sharing the risk. This is a multi-cycle plan (3 Fresh cycles are offered, each with 1 Frozen Embryo Transfer if needed) designed to limit cost by placing significant risk on the Center if an on-going pregnancy is not attained. The end result is containment of cost with all costs known before treatment begins. Those couples wishing to enter a Joint Venture Program must have normal ovarian function (determined by the patient's ovarian reserve profile) and the woman must be less than 38 years of age. Semen parameters must not be lower than a 10 million Total Normal Motile Sperm Count or less than a 10% normal morphology. There must be no more than one previously failed IVF cycle. The woman’s body weight must be 95 – 120% of normal. Pre-implantation Diagnosis (PGD) and Single Embryo Transfer (SET) disqualify couples from the Joint Venture plan.


Joint Venture with a Refund for No Take-Home Baby
The Joint Venture with a Refund for No Take-Home Baby Plan represents a higher level of risk for the Center and a 70% refund of the basic cycle fee if there is no take-home baby, therefore, a single fresh cycle with a single frozen embryo cycle is offered. Each couple must be qualified for each cycle. Being accepted once does not automatically mean they will be qualified for another cycle. Women must be less than 36 years old and meet the other qualifications listed under the Joint Venture Plan above.

Donor Egg Cycles
In Donor Egg Cycles, since all egg donors will have proven ovarian function, all cycles are qualified when semen parameters are not lower than 10 million Total Normal Motile Sperm Count and at there is at least 10% Normal Morphology. Donor Egg Cycles are available as either Fee-For-Service, Joint Venture Plan (3 fresh and 3 frozen cycles) or Joint Venture with a Refund for No Take-Home Baby (1 fresh and 1 frozen cycle) plan.

Insurance-Covered Plans
For Insurance-Covered Plans, all fees will be charged to the insurance company per CPT code. The patient is responsible for co-pays and deductibles.
