Before ordering donor specimens from Procreative CryoBank you must open an account. Please be prepared to supply the following information every time you place a specimen order. Specimen orders must be placed by telephone during our business hours.  

1. Your account information: Customer name on the account and you may be asked to provide your mother’s maiden name or SSN.
   
2. Donor number
   
3. Specimen type: IUI or ICI
   
4. Quantity: The number of vials you will be ordering
   
5. Shipping: The type of shipment you will need ( Pick-up, Courier, Fed Ex Priority Overnight).
   
6. Date of Pick-up/Shipment:  The date that you need to pick-up the specimen or have the specimen shipped. 

Please call in advance to place an order due to restricted number of tanks available.
