Normal business hours for Procreative CryoBank are Monday, Wednesday and Thursday from 8:30 a.m. to 4:30 p.m. and Friday from 8:00 a.m. to 4:00 p.m. Weekend appointments can be scheduled for Saturdays and will be made for emergencies only. Saturday appointments must be made in advance (at least one week prior) and an extra charge of $100 will be assessed to cover the cost of staffing the laboratory. Tank rentals and specimen pick-ups will not count as an emergency, so if your insemination procedure is expected to fall on a weekend, please pick up the semen sample on Friday. THE LABORATORY WILL BE CLOSED on all Sundays, major holidays, or holiday weekends.
Specimen Repurchase Policy
Procreative CryoBank clients may be reimbursed half the original price for unused donor specimens that were purchased at regular prices and that have never left the Procreative CryoBank facility. The repurchase must occur within 18 months of the original purchase date*. No more than 6 vials can be returned for the partial refund. Please note that Procreative CryoBank will not repurchase donor specimens that have been transferred from another facility or that have left the Procreative CryoBank facility at any time.
*No refunds will be given on any storage fees paid for specimens that are being repurchased by Procreative CryoBank.
Client must complete the Specimen Repurchase Agreement form issued by Procreative CryoBank at the time of the repurchase in order to receive any refund on repurchased donor specimens.
Specimen Return Policy
Procreative CryoBank will place unused specimens back into storage if the specimen has not been removed from the rented/shipped tank. Due to AATB restrictions and risk of contamination, Procreative CryoBank cannot re-store specimens that have been taken out of the rented/shipped tank at anytime, unless they have been stored at an accredited laboratory.
If for some reason an unused specimen needs to be returned to Procreative CryoBank please call first and notify one of the lab technicians so that the tank is checked immediately. Also, include a written note inside the shipping box for our records. There will be a $50 charge for the extra handling of the specimen. If the lab is not given advance notification of the returned specimen, there is a risk that the specimen may not be immediately returned to storage and may thaw. In this case, no refund or credit will be given. Procreative only guarantees that the shipping tanks will keep specimens frozen for up to 7 days. If a tank has been out for more than 7 days no refund or credit will be given for thawed specimens.
Tank Return Policy
If there is no one in the office at the time of the tank return, you may leave the tank in the waiting room. If a tank is brought back during hours when Procreative is not open and is left in the hallway, you will be charged $50. Leaving a tank in the hallway increases the chances that it may be damaged or stolen. If a tank is damaged or stolen and has not been properly returned, you will be charged $700. These tanks are very expensive, so please take care of them. Tanks must be returned by the due date on the Packing Slip. There is a charge of $10/day for any late tank returns.
