FAQ’s
- How do I get started?
We suggest that you consult your physician before ordering a specimen. Your physician will help you decide what type of specimens you will need and will guide you as to which type of insemination is best for you. Once you have selected your inseminating physician, you must complete and send us the Authorization for Release of Semen form (Exhibit P1).
- How are the semen specimens packaged and shipped?
Specimens are shipped in special containers that contain liquid nitrogen. Our shipping containers are designed to maintain the proper temperature for up to five days. Some specimens are shipped in “dry” shippers; therefore you may not see any liquid inside the tank
- What methods of payment do you accept?
We accept payment by check, money order, cash and most major credit cards including Visa, MasterCard, and Discover card. Payment is expected at the time of service. Procreative CryoBank does not accept insurance of any type.
- Can I perform the insemination at home or can this only be done at the doctor’s office?
We can send your donor specimen to any address specified on your new account form. We do require our clients to be under a physician’s care. Your physician must sign an Authorization for Release of Semen form (Exhibit P1) form before you may purchase any specimen. We recommend consulting with your physician on all matters relating to inseminations, but the decision to inseminate at home is between you and your physician.
- How do I order the short and long profiles?
Please call our office at (314) 997-3620 to place orders for short and long profiles.
- Whom do I notify to report a pregnancy?
All clients should receive an Outcome of Insemination Report with their specimen shipment. Clients should complete this report and mail it back to us.
- What type of storage terms do you offer if I want to purchase and store sperm for a particular donor?
PCB offers storage terms for purchase and store accounts ranging from 6 months to 2 years. Purchase and Store accounts are a great way to ensure that vials from the donor you have selected are available for both current and future family building needs. - Does California CryoBank offer a Buy Back Program?
PCB does offer a Vial Buy Back Program. Please see our Specimen Repurchase Policy for details.
